PHP has a Diabetes Management Program to assist our members diagnosed with diabetes and to offer encouragement to manage this condition. Just click on the Get PHP Fit! icon, and select Get Fit Rewards.
Members are rewarded for following doctor's recommendations for the preventive tests HbA1C and retinal eye exam. Members receive an e-gift card incentive for completing each of these tests annually.
You must be an actively enrolled PHP member at the time the incentive is earned, and have a diabetes diagnosis to participate in the program. We will make an investment in you as you make an investment in your health!
